Except it doesn't even take a second for the password input to appear. Where are you getting those numbers from anyway? When I tried it, it only took about 152 ms for all the data to load after submitting the e-mail address.

That's not true, though. You would still have to either press tab (now enter), or click the second field to enter the password.

You can use any other TOTP-based application instead of Google Authenticator. Plus, unlike SMS, it works offline.

That's absolutely not how it works. Firstly, "www" is a subdomain of "example.com". The subdomain could match the hostname of the server that hosts it, but that's rarely the case (there can be multiple servers

Because encountering a site with a self-signed certificate likely means that the connection is MITM'd. In any case, with a self-signed cert you can't who you're connecting to.
