Which source? Bitcoin is a network of computers speaking a common protocol, not one implementation. Yes Bitcoin core is governed by core. If you're not happy with core then go run a different client or go write your own. You don't

You don't need a permission slip to write your own client with whatever rules you want. You do need to convince people to use if you want it adopted. What do you mean that it's centrally planned?

Take a look at the discount for segregated witness transactions. You'll see that in a world with segwit there are more transactions per block than without.
