That doesn't say that they pre-authorize the amount. The requirement they're probably referring to is the ability to send funds directly to the project creator.

Because some parents might disable Safari but still enable installing apps that have appropriate age ratings, and installing this app is a way to bypass Safari being disabled. (Not that I agree with any of that -- that's just why).

This is why you shouldn't allow GET for performing actions. An image tag in an article could do the same thing (e: if it didn't check the referrer).

No site served over unencrypted HTTP can be considered trusted. So there's no circumstance under which they should insert this header, since they can't modify HTTPS requests.

Say if Facebook donates a million dollars a year, Wikipedia would be incentivized to treat Facebook favorably. Obviously this doesn't happen, but that is one of their arguments against advertising.

Right, on iPhone 4 it would be 39 pixels but on iPhone 3GS and lower it would be 19.5 pixels, which would require the icons to be aliased and blurry.
