You don't need to enter your phone number to use Google's two factor. You can use their smartphone application to generate codes. If you don't want to do that, the algorithm is free and open-source so you can probably find an alternate impl

Looks like the crash doesn't happen if you provide the </form> tag, so that looks like it. Edit with more info: Looks like removing any of the markup prevents the crash. "Removing the CSS, the div, the table, etc all cause this code

As the defender, you don't care if the salt is obtained by the attacker. The salt is not a secret. It's only use is to ensure that each password is hashed as unique, even if the users chose the same password. Basically, it is for defeating

I think you are right about the gmail issue. I use adium on an iMac and Macbook for work and Digsby on my Windows box at home and the application specific passwords work fine for me.
