I've built this a few times as well. They all revolve around known patterns, https://en.wikipedia.org/wiki/Slowly_changing_dimension I highly doubt there's a single way to do this that solves every scenario.

No just higher fees but the merchant becomes liable for fraudulent transactions processed without chip and pin. Whereas before the big card brands would cover more the risk and fees.
