We still need there to be suitable alternatives, so there needs to be some regulation at least, to encourage competition and/or to prevent companies from reaching a point where there are no longer alternatives.

My gut instinct is that the mob might be a (potentially significant) factor here. I could be wrong, but this is their bread an butter after all: bribe politician to get large-scale infrastructure project and then artificially inflate costs
