Any sort of versioned online wiki tool is better than word documents with filenames - this can be your own wiki, or sharepoint or Drive. Non tech savvy users can read the page to see the latest version, and others can track through history

The most important way to do this is to actually be a user of your product, e.g. "Eating your own dog food". This doesn't mean logging in, trying all the features and playing with it for an hour - you have to really use it

They certainly wouldn't be calling lots and lots of API's, as that would likely trigger some antivirus software or at least leave traces in system logs. The sophistication of this software is that it did all this undetected.

I don't know, but I just assumed they use it everywhere. They have a very good mapping of related terms and a fairly consistent mapping. A visible example is when you look for organisations and they have a classification against it. e.

Yes, if you already know SQL and by using bind-parameters, then I [personally] can't see a tangible benefit for using an ORM. They can certainly make for cleaner looking code, and are useful for people who don't really understand

Yes, this is probably the biggest issue, and mainstream media don't help by mostly publishing 'bad GMO' stories where no one hears of the hundreds of success stories (in fact you could apply that to most of the sciences)

If the database changes are substantial enough to break the applications, I would have a new schema setup to have enough time to test carefully and then when ready, point the application to the new schema. For small changes, we use scripts

I don't think the main issue is what tool you use is, but rather where your master copy lives. I was vigilant using txt/docs in a nice structured folder setup on a local server, but as I started using Google docs / Evernote
