  I was in the ICU for a month after a motorcycle accident in the '90s. I didn't see all the bills, but one of them was for over $300K. I had the "bad" insurance at work, which had bigger co-pays for day-to-day stuff, but happened to have no max on coverage. I paid nothing.

      It was presented as the worst, because the day-to-day costs for things like doctor visits and x-rays were higher. My case was unusual.
