  My point was that when the plane is pointed at the ground, that won't help.

    Pick up the fire axe that is also typically present in the cockpit and kill the person flying the plane. Then open the door.

      seems like a potential suicidal pilot would have quite a chance to get the drop on the flight attendant...

    It takes a quite long time to get a big plane "pointed at the ground". It's also impossible to get a plane literally pointed at the ground without breaking it, and most planes can not maneuver enough to get there. In high altitudes, there's plenty of time to open the door, immobilize a rouge pilot and fix things.
