For what it’s worth, I searched for an alternative and ended up finding deftform.com on appsumo with an affordable lifetime subscription. I’m very pleased as it replaces a bunch of other apps I was using like Google Forms and hellosign.

Update: Because they banned everything associated with my account, my developer account has been disabled, which broke a live mobile app. I have no way to restore or fix it because they seem to have blocked my IP from creating new accounts.
