I’d recommend Normal Accidents by Charles Perrow. I no longer believe the impervious system is possible. I do think PG&E needs to be forced to invest more in the infrastructure, but, at the end of the day, these accidents will still hap

Your point is well taken, but it may still be necessary to know others - for example, I work at a large and well-known Internet company and we use Perforce =)
