That's how every dishwasher I ever owned used to work. And it's actually a feature: you can add stuff that you forgot to put in after it starts, and you can take something out, when you need it immediately, and wash it manually.

Actual conflicts from working with the same fields in the same tables are so rare, I haven't even experienced it once. However, the second scenario would happen every single day in a team.

When the question is about 100% pair programming I'd say there aren't that many. Hashrocket is one of the few companies (I believe Pivotal is, too) where it seems to be true, but most teams that I know are only pairing part of their time.
