Safety is not really about reliability. Maybe as a means to an end. It's about not killing people and maybe not destroying too much of your facility in case things go south. Safety systems take over if your chemical reaction overheats

Regarding firmware patching, look at InternalBlue [1]. I guess they did it for one IC, but obviously can't do it at will for just any IC and instead hand wave "it can be done". Overall I don't think it's impossible
