I'd like to think that too. But then I think: If a company had a legal right to fire me whenever they wanted, and I had a mortgage or family, I wouldn't work for them unless they agreed to waive that right and give me several months notice

What about embarrassing problems with already-known fixes? Or easily-diagnosed but hard-to-cure embarrassing problems? I can't imagine a patient would benefit from the world finding out they had syphilis.

Also if you did it db backed in a separate table, you'd have to do database joins to retrieve avatars, which might have a performance impact.
