I agree this mistake is typical, but I would not be surprised to see a senior make it. A related mistake is, of course, to keep a pilot project in production (usually as the core of the production project, with a variety of both functional and structural additions to keep it sort of working). This mistake is usually a result of a management decision more than development (but could be the same person).
