If John selects male on the census check box, dresses as a male, and tells everyone publicly that he is a male, there is just no way to know that he really wishes he were female. He flat out said he'll check male unless it's an anonymous survey--and even if the survey is anonymous, he said he'd check male as long as anyone else is in the room with him.
