Especially in teams, one thing I've found that helps in documentation is standards and templates. If everyone knows exactly what to document, how to document it, and where the documentation lives then it's much more likely to get done and done well. Start with a minimal set of standards and templates and build up, rather than over-engineering and not getting anything done.
