You would think Indians are lucky in this regard. It's actually the opposite, despite being a software developer who earned reasonably well (compared to the norm) I dread the thought of going to a good hospital. I just don't have the sort of money they ask for. I cannot even imagine what it is like for a poorer shopkeeper or factory worker. For comparison, I earned $1000/month after 6-7 years of experience. $1700 after a decade of experience and ability to program in 3-4 languages. Most jobs will earn you around $500-$600/month. That is why in India people save money because the parents tell them from childhood that hospitalizations / accidents can cost a lot of money so better save up in case you get sick.
