Ask HN: Do any of you use a personal medical CMS and database?

I have had my fair share of medical issue - none life-threatening - of which some still endure, and my experience with doctors is that they sometimes fail to send and share my medical data between them. Because if this in part, I have found it necessary to create back-ups of my medical history, which involves scanning paper documents most of the time. I just have a folder with a set of subfolders now, but I wonder if someone has found a way of managing their information conveniently? Preferably locally or self-hosted without relying on anything external. (For what it's worth, I do not live in the U.S.)
