Ask HN: What negatively affected your team's progress in 2013?

Was just talking to some friends (we all work for software teams). We were talking about how each of our teams had done in 2013. A few pains that kept coming up: 1) Hiring: finding people, hiring the right people, managing conflict 2) Getting stuck in a development rut: long projects that never end, slow progress, the need to re-build features 3) Staying focused on the customer: building what they actually need, remembering why you're building features, etc... Have you seen similar things in your startup? What affected your team's velocity in 2013?
