It's very obviously not built by google. For starters the map is done wrong. The logo is old. I could go on but I think that is enough. But I also don't think it's a phishing attempt. What they "collect" is basic information that a simple first name last name google can return for most of the people who fill it out. And to top it all off, they have a radio button that says "By submitting this application you agree too ..." that actually has to be selected +_+
