I'm not really seeing a "problem" here - uniformity in standards is a recipe for stagnation. There's no need for everyone to do everything the same way, and even less need for prescriptive standards that attempt to change people's behavior than descriptive ones that just document what people are already doing. I'm a bit confused about the paper sizes thing, though A4 and Letter are simply two different paper size standards.
