My dad with asthma has severe problems in the winter because everybody in his village heats with wood - in the middle of western Germany (they do have other heaters, but like the wood, is what I mean). So definitely not just a third world problem. In Germany the heating with wood was extremely on the rise in recent years, even in big cities you smell it on the street in winter. And in the countryside it is much worse, which is a shame given that people move there for the clean air, among other things.
