2) Same as 1. Standard is 25-30%.
3) Some of the best hires at firms I've worked at came from recruiters. From the candidates' point of view, it's often a lot more efficient to find matching positions and land offers through recruiters.
4) Same as 3. Loyal employees use recruiters too.
