I worked at a place where I was the only person responsible for the game dev section of the company, about 50 people. On my second day I discovered that QA listed a bug as high priority with a "1", while the developers put a "10" on the highest priority. I think you can see that there was an issue. They had operated with this system for more than a year before I got there. I switched them to a letter system, "A" being top priority, "B" is second etc on my third day. Their world changed.
