Why did Kaiser’s insurance not pay out here?

  It did, but my contribution for procedure + pathology was still over $5k. My out of pocket max is ~$8k.

    Oh, OK. But you didn’t pay 17K yourself? (I’m also on Kaiser, but never had any major procedure done.)
