So access to healthcare probably plays some role, but it seems like there are lots of other confounding factors. It also doesn’t seem to vary that much by state level healthcare differences. States vary quite a bit by geography, but states that didn’t do Medicaid expansion, like Wisconsin and Wyoming, seem to have similar rates to neighboring states that did.
