Regarding the policy violation, is it possible the problem was the fact she lived in Norway and was using amazon.com? My experience, as a Canadian, is that Amazon encourages use of amazon.ca (your local version of their site) and you have to go through some tricks to use Amazon.com. Many of my friends did because that was the only way to get a kindle and books for years. Are they all at risk of this?
