Or society could decide that people shouldn't have to pay for surgery and healthcare should be provided to all without them having to cut a cheque.

Having a "single" "government" payer where claimants actually pay nothing at PoS can be thought of like a very low deductible health insurance plan, can it not? Everyone still pays their premium in taxes, unless we mean to enslave doctors and nurses.
