All of this needs to be traced back to the source driving all actions: incentives. People will do what they're in incentivized to do. Yeah, it's a "duh!" statement but needs to be front and center when analyzing workflow. Of course incentives can be gamed as well, so they should not be accepted blindly. There should be a Department of Game Theory that could help analyze and adjust accordingly to "do the needful for the same".
