My out of pocket pre-insurance costs in Portugal are so low that I sometimes forget I still need to file for reimbursement with my private insurance provider. My copays on certain plans in the US were higher than this. There's no way the cost difference (often literally 10x+) in private healthcare with the US can be explained by anything other than "US prices are actually total bullshit".
