- rscho 1y agoThis time it killed someone. Usually, it's just a bed or a respirator that ends up stuck in the ring.

- bookofjoe 1y agoDeaths in the OR like cardiac arrests, fatal hemorrhage from burst aneurysms, etc. are always reported within the hospital. Whether others outside learn about such things is often a matter of persistent family and relatives demanding to see the actual death report and contemporaneous notes. Fatal mistakes usually stay within hospital departments and are discussed at length in regular confidential Morbidity and Mortality conferences.
