My thinking is threaded too, but it's more like the XKCD primer plot diagram. I have a churn of tasks and subtasks that I add do, my brain randomly picks one to be the most important thing in the world and I do that to the exclusion of all else. Methylphenidate didn't help much but dexamphetamine seems to improve things a bit once I get past the drowsiness it causes.
