Another big piece I would add to this is the processes that enable organizations to ship code. Not just processes that are directly related to the product and code, but other organizational processes like hiring, sales, support, etc. Efficient processes require a lot of thought to develop and implement. When a badly-run organization acquires a good piece of code, it will eventually start to stagnate and bloat.
