"A lack of vigilance and discipline" is a good diagnosis, and "write code with vigilance and discipline" is good advice. Now, of course that does not spell a plan that you can blindly follow step by step, and that's good! It forces you to think about what this means for you and your team and to come up with strategies and processes that will fit your needs. This should not be too difficult for people in the field. It might sound basic or obvious, but actually most commercially projects I have been involved with lacked both. It takes a firm leader to instil and maintain both.
