We should be worried about all of them. But a hostile totalitarian foreign government could have motivations that are a hell of a lot worse than maximizing engagement/profits. If the goal is to cause harm to the population (ala fentanyl distribution) rather than just to make as much money as possible, I’d say parents are right to be correspondingly more concerned.
