This makes no sense. Plenty of healthcare is not covered by insurance. Cosmetic procedures are healthcare and not covered. Not to mention errors in claims, claims that include care well outside guidelines, etc. You’d expect a pretty consistent level of denials even if all medically necessary claims are approved.
