You can expect it standard on a restaurant burger or a premium quick service place, but not standard for normal fast food and certainly not McDonalds. Even at more upmarket and expensive fast food like Chick-fil-A, lettuce and tomato is not standard but rather a "Deluxe" option that comes with an increase in base cost.
