Bad requirements are the biggest productive killer.

  It's complicated. It can become an excuse too: "if only they (waves in the general direction of customer relations) could give us good requirements". Some stuff is just difficult to figure out.

    Also often times the customer does not know what they want, they only know that what they have isn't it.
