The first 2 and last incident seem like mapping errors. I know people working in this space, and it sounds like they all think the problem is "just giving it a better map". They want to rely primarily on a world map and use things like vision to "see" cars and other local stuff. That approach will never work (except in limited cases like Waymo mostly gets away with). Humans rely primarily on vision and understanding, and reading signs. FSD requires AGI. These systems are all expensive science fair projects and have already killed a number of people.
