On mobile, I frequently bump the "flag" button by mistake. Often it happens quick and I don't notice it. I once checked my list of flagged topics and they were all pretty anodyne, inoffensive, constructively posted links I don't even remember flagging. So I think it may be a UX issue, potentially remedied by a mobile-purpose stylesheet with larger UI elements.
