In many cases (not sure of the rules), it fiddles with the case, as it does various other title manglings. If it does mangle things, the submitter can edit them back the way they were and it won’t mangle again. I strongly dislike the title mangler and would fain see it removed. I feel like I disagree with its modifications most of the time, and even the ones that have some justification like removing numbers at the start as a declickbaiting measure (“400 ways to do X” → “Ways to do X”) very often harm the sense, and I dislike these cases more than I dislike its improvements. (But maybe if the mangler were disabled I would realise that people are doing a terrible job of entering titles and it is actually fixing a lot of stuff.)
