The perhaps more informative framing is: Why do doctors in America earn so much more than their peers in other countries (i.e., those with roughly equivalent systems). Pulling on that thread is pretty interesting in how broad it goes. I'm certain I don't understand it well and the experts are pretty argumentative also.
