- 5555624 3y agoTo prove a permanent disability, one option is a statement by a licensed physician, which must include: - that the individual has a PERMANENT disability - that it limits one or more aspects of their daily life - and the nature of those limitations. Unless there's something about the nature of the limitations deafness seems like it would apply. (The other acceptable forms of proof are federal or state government-issued documents.)
