I use a hybrid approach. My main database and app is still centralized. It doesn't need hyper optimized performance. But it does push certain data to Cloudflare KV that I serve up with Cloudflare Workers. That is data which is read only and needs to be optimized for fast global loads. Really depends what you're building.
