McNamara's Vietnam War-era career is, in general, a long-ass list of well-meaning, manufacturing-inspired data-driven decisions and efficiency drives (after all, he was previously president of Ford) that horifically backfired. I wonder whether there's a "Mythical Man-Month" style book about that out there.
