In my experience, this is the vast majority of cases. Hence:

And to make matters worse, those seem to be the same environments where businesspeople make target dates without engineering, and then get angry when the deadlines aren't met. Put another way, poor management is a human layer problem. No technology (whether process or tool) can solve that for you.
