I've seen this behavior on sites with CSPs that'd break the challenge, They somehow get loaded from cache and cause failed requests. This somehow even persisted into the browser's incognito mode, and I had to use an entirely different browser. This wasn't on a small unknown site either. (It looks like pinned CSPs are a dead standard, but did anyone implement it?)
