I've certainly run into the yak-shaving problem of having to chase a hundred things down to get one thing fixed (this current job of mine is full of that shite) but the real productivity killer when my path is clear is myself; the stupid ridiculous little bugs I introduced by accident that I can't see because they're my own they can cost me hours. And I generate very regrettably large number of them so I'm constantly tripping over myself.
I keep reading what I expect to see, not what is there. If anyone has some advice on how to be less stupid in this aspect, I greatly welcome it.
