Maybe the author wanted to be offensive, and we just didn't realize, IDK But as you mentioned, the author's estate, family and editors that worked with them when they were alive are the ones that can probably make the best guess. My problem is when publishers make changes even at times when authors are explicit in saying they don't want changes, without (not sure about this specific case, I'm just assuming here) getting approval from the estate
